Service Coordination
Service Coordination uses proven steps, in which an adult (starting age 18) or family of a child (3-17) is linked to services within the community to support his/her individualized needs. Crisis Assistance is also available only after a treatment plan has been approved.
Case Management
Service Coordination offers Medicaid services for adults (starting age 18) and children (3-17) diagnosed with severe and persistent mental illness. Service Coordination for adults assesses financial, housing, vocational/educational, social, legal, family and basic living. Service Coordination for children assesses the following needs: utilizing family centered planning, thereapeutic, educational, social/community integration, personal, long-range planning, legal and financial needs.
